Xpressions Community Living Solutions (XCLS) is a community-based provider dedicated to delivering compassionate, person-centered support to individuals and families in the comfort of their homes and communities. Our mission is to promote independence, dignity, inclusion, and overall well-being by providing high-quality care tailored to each individual’s unique needs and goals.
At XCLS, we understand that every individual deserves the opportunity to live a meaningful life within their community. Companion Services
Providing socialization, supervision, and assistance to promote independence and community engagement.
Homemaker Services
Supporting individuals with household tasks to maintain a safe, clean, and healthy living environment.
Community Participation Services (CPS) (LPN & CNA Behavioral Health Specialist) –
Facilitating meaningful community involvement, skill-building, and social integration.
In-Home & Community-Based Nursing (LPN & CNA) –
Delivering skilled nursing care and personal assistance in home and community settings.
Family Medical Support Assistance (LPN)
Partnering with families to provide medical oversight, training, and hands-on support to ensure continuity of care.
Behavioral Health Specialist Services –
Offering behavioral interventions, support strategies, and individualized plans to enhance emotional and behavioral well-being.
Respite Services (LPN & CNA)
Delivering short-term relief for primary caregivers while ensuring skilled and compassionate care.
Transportation Services
Providing safe and reliable transportation to medical appointments, community activities, employment settings, and essential errands to ensure access to services and community integration.
Employment Support Services
Supporting individuals in obtaining and maintaining competitive, integrated employment through job development, job coaching, workplace skill-building, and employer collaboration to promote long-term success and independence.
